Reporting accidents, incidents and near misses
Please use one of the forms below to report health and safety events at work.
This helps us improve how we work and keep people safe.
Choose the right form for the accident, incident or near miss you want to report.
Use the guidance below to help you fill in the correct form.
Accident: Something unexpected that happens at work and causes injury, death or ill health to an employee or a member of the public.
Incident: Something unexpected that happens at work and does not cause injury, but may cause damage to property or equipment and still needs to be reported.
Near miss: An event that does not cause harm, but could have caused injury, ill health, or damage to property or equipment.
Please complete the form as soon as possible, and within 24 hours of the accident, incident or near miss. Include as much detail as you can and fill in every section of the form.
When you submit the form, it will automatically be sent to safe@acisgroup.co.uk for review. We will then decide if any further action is needed.

Health and safety is a core part of everything we all do. We rely on all of our people to play their part in implementing our health and safety policy and drawing to our attention, areas in which we can improve.
To that end, and to comply with health and safety legislation, we are all required to read the three documents that make up our health and safety policy every year.
Part of 'your part' is knowing our intentions to create a safe and healthy culture, who is responsible, and the arrangements we have in place.
Please allow approx. 10-15 minutes to read this information.
If you would also like a printed copy of the documents, just download and print or ask a colleague / your manager to do this.
At the end of this training, you will understand the risk associated with working on houses with gas and other fossil fuels.
